Program/Service Notes:
DSCC operates the Home Care Program on behalf of the Illinois Department of Healthcare and Family Services (HFS). The program serves families who care for children and youth who require skilled in-home nursing. Many eligible youth are also medically fragile and dependent on technology for their well-being. Home Care services help youth remain in their family home rather than a hospital or skilled nursing facility through care coordination, connection to service providers, paying for home modifications, etc.
