Program/Service Notes:
Program provides a wide range of options to help individuals live as independently as possible in the community. Supports can include money management, hygiene training as well as case management services. Program also provides service faciltation services to adults and children in the Home Based Support Program. The Client & Family Support program provides services to individuals funded to participate in the Home Based Support program, Family CILA or Intermittent CILA program. The program also coordinates the services of the Respite program grant. All services are geared towards assisting the individual to attain or maintain their home in the community.
